Application is hereby made by the undersigned to the Town of Horseheads for permission to construct, alter, add or locate a driveway in the following town road. (This includes re-paving of entrances.)
 

Said construction is to be made in accordance with the specifications and requirements of the Town of Horseheads Highway Superintendent. Also, the Highway Superintendent may impose any special requirements witch the particular situation at the location where such driveway is sought to be located requires in his or her judgment under the circumstances.

REQUIREMENTS:
1. Applicant shall bear all costs in the Town right of way, and meet all conditions set forth by the Highway Superintendent.
2. No more than two driveways to a commercial establishment entering one highway.
3. Maximum width of residential driveway is 20 feet.
4. Maximum width of commercial driveway with one entrance is 50 feet and with two entrances is 35 feet.
5. The angle of the driveway shall not be less than a 45 degree angle.
6. No driveway within 50 feet of intersection.
7. No driveway will be permitted when site distance is less than 350 feet.
8. Culvert pipe shall be no smaller than 12" and no shorter than 20 feet.
